Exit Questionnaire

Queen Mary values its staff and recognises the importance of understanding why employees leave the University.

The exit questionnaire is a useful tool for collecting information at an organisational level in an efficient and accessible way. This does not, however, replace the exit interview and we do still encourage managers to informally meet with staff so that they may gather local feedback to inform local decision-making. We also encourage staff to have open and honest discussions with their line manager or an alternative manager before they leave.

The exit questionnaire is available as an electronic survey to be completed online.

It is also automatically sent to staff who have given their notice to leave. This will happen once HR have been notified of the leave date and have entered it onto the HR system, which will then trigger an automated e-mail to the leaver with a link to the questionnaire. Submission of the questionnaire will be to the survey host.

If an employee does not have access to a computer please print and fill in the Exit Questionnaire PDF [PDF 184KB] version by hand and submit it to HR reception at Mile End.
