The University is updating health assessment criteria as new research and guidance becomes available. If you have completed an earlier version of this assessment it is not necessary to do this again.
As the University continues to open more of its campus buildings and plan for the safe return of its staff and students in a phased manner, it is important that we ensure that staff are safe and that we are able to provide assurance and confidence to all staff about their return to campus. We recognise that some staff may still feel anxious. This guidance is designed to ensure that our return to campus risk assessments, the processes to identify and mitigate any risks, are carried out in a genuinely inclusive and person centred manner. There needs to be a collective responsibility from managers and staff to identify all necessary actions to support returning safely. This will be the key premise and approach of our risk assessment processes.
The Individual Health Assessment Form [DOC 243KB] should be completed by all staff either currently working on campus or expected to return to campus.
Managers and staff should familiarise themselves with the Process Guidance [DOC 394KB] which explains the assessment process and how to request an occupational health referral if required.
Staff must notify managers that they have completed their form. They should only return the completed form to their manager if their assessment stipulates a referral is required.
It is recommended that staff share their completed form with their line manager via OneDrive in order to maximise security.
Where a member of staff has expressed a concern regarding sharing the form with their line manager, this should be referred to the line manager’s manager in the first instance. Guidance may also be sought from the HR Partner for the area.
